Passport Information
Passport or Passport Card - What do I need?
The main difference between the passport and the passport card is how you can use the two official documents. The passport is good for any kind of international travel. A passport card is only good for travel within the continent of the North and South America; therefore it is good for crossing into Canada or into Mexico, anyplace that you can drive to from the United States. But you cannot use a passport card if you are flying to a destination. It will not be accepted. For me I chose to get a passport just in case but for you you may want to choose to save the money.
If you are 17 and Older and need a passport do the following:

Go to Walgreens and get your photo taken
Go to http://www.state.gov/documents/organization/212239.pdf and print out the form and fill it out
Take it and go to Modesto Public Library, on I street. They are super nice, there is never a line and they are helpful.
It is better to do this sooner than later, so do it now. However do not panic, they always will give you the worst case scenario in terms of how much time it will take. I have always found it to be faster.
If you are 16 or younger follow these instructions:

Traveling by Land?
All adults arriving by land from Canada or Mexico must have a passport and either a passport book or passport card may be used.
U.S. citizen children under age 16 arriving by land from Canada or Mexico may present an original or copy of his or her birth certificate, a Consular Report of Birth Abroad, or a Naturalization Certificate.

Mexico Mission Trip Application Form
The Mexico Mission Trip from Calvary is about the following...
- Practicing servanthood, by serving others.
- Practicing discipleship, by following Jesus
- Practicing loving, by living in a community of love while showing love to each other.
- Practicing the acronym: FATW [Explained below]
To accomplish this, we use the Yugo team in Ensenada, Mexico to build homes for people and for building relationships with kids. It is an intergenerational team made up of adults, families, and youth.
Requirements:
- You must be physically able to go. [This means that you can handle a 14 hour drive in a car, work in the heat and sun, deal with diminished sleeping hours, and have a flexible diet.]
- You must be 13 years old to attend the trip without a parent. 12 and younger must have a parent on the trip.
- You must fill out the application and have it approved.
- You need to participate in the fund raisers or pay the full amount of the trip.

Our fourth purpose for this trip is to practice FATW. This acronym stands for four words that absolutely must be practiced on any mission trip or you will not enjoy the trip and neither will anyone around you... [Hint, Hint]
F- stands for Flexible. We must let go of our ideas of what should or should not happen, let go of what we want to do/want and go with the flow of the moment. Not always asking what is next, but be...
A- stands for Available. We must be available, this happens when we set aside our own agenda for what is going on around us. We five in the moment, rather that living for the next moment. We are fully present in the hear and now. We notice what is going on around us. We are not so self absorbed that we do not notice the needs of others.
T-stands for Teachable. None of us are the experts. We strive to be life long learners. We may know more that someone else, but that does not mean that they have nothing to teach us.
W - stands for Willing. We must be willing to do the work that is asked of us. Many times it may not make sense, it may not be to our liking, it may not be fun, but that is not the point. The point is that we came on this trip to Serve, Disciple and Love. No one made us come on this trip, so have a willing attitude will make all the difference.

In consideration for the below named being accepted by Calvary Reformed Church for participation in
the _______________________________________ Event, ________________________, I (we) being 21 years of age or older, do myself (ourselves) and for and on behalf of my child-participant (if said child is not 21 years of age or older) do hereby release, forever discharge, and agree to forever hold harmless Calvary Reformed Church, its pastors, officers, youth leaders and agents thereof, from any and all liability, claims, demands for personal injury, sickness, death, as well as property damage and expenses of any nature whatsoever which may be incurred by the undersigned or the child participant resulting from said child's participation in the above described trip (including travel while participating in the mission trip.)
Furthermore, I (we)(and on behalf of our child-participant if under the age of 21 years) hereby assume all risk of said personal injury, sickness, death, damage, expense as a result of participation as set forth above. Further, authorization and permission are hereby given to said organization to furnish any necessary transportation, food, and lodging for and to assign work projects to this participant. The undersigned further agree to hold harmless and indemnify pastors, officers, youth leaders, and agents for any loss claim, liability, damage, including property damage, or injury whatsoever incurred by the child-participant as a result of the negligent, willful, or intentional acts of said participant, including reasonable attorney's fees and other expenses incurred attendant thereto.
I (we) am (are) the parent(s) or legal guardian(s) of this participant, and hereby grant my (our) permission to take said participant to a doctor or hospital and hereby authorize medical treatment, and assume the responsibility of all medical bills, if any, in excess of any applicable medical insurance coverage provided through this treatment.
Further, should it be necessary for the participant to return home due to disciplinary action, for medical reasons, or otherwise, I (we) hereby assume all transportation costs. This authorization is given pursuant to the provisions of section 25.8 of the Civil Code of California.
Both parents must sign unless parents are separated or divorced, in which case the custodial parent must sign.
